Six-year-old Zohar Dillard fell out of a window of the fifth-floor New Jersey apartment where he lives with his mother, Alexis Adams, Access Hollywood has learned. According to court documents obtained by Access on Thursday, the incident took place earlier this month and the boy sustained "severe and permanent injuries" after landing on concrete.
